FAQs

What To Wear & Bring

Please wear clothes that you feel comfortable exercising in and that allow us to easily access the injured region of your body. Feel free to bring any medical records or imaging (X-Rays, MRI, CT Scan) that you feel will allow us to more effectively treat you.

Click the tab to the right to download your New Patient Intake Forms and please fill them out prior to your first visit.

What To Expect

Rehab Renegade Physical Therapy and Performance is unique in the sense that it is unlike most physical therapy clinics. You will always receive 1:1 treatment from a Doctor of Physical Therapy. You have our undivided attention during your sessions and will never receive treatment from an aide or technician.

You will receive a thorough evaluation on day one in conjunction with hands on manual therapy as needed. We will work as a team to create a plan that will allow you to achieve your goals, whether that is pain relief or performance.

Do I Need a Referral?

The Direct Access Law in California allows a licensed physical therapist to provide physical therapy services without a referral. In some instances, HMO plans will not provide reimbursement for sessions without prior approval.
